From Tbilisi take an intercity minibus/bus from the Didube or central bus stations to Gori (about 1-1.5 hours), then continue to the village of Ateni by local taxi or minibus (the village is roughly 10-15 km from Gori); from the village centre the church is a short walk (around 5-10 minutes).
